                   2)   This session is only open to bowlers with averages of 201 and higher.
                    3)   Bowling the week of 10/19/09  through 10/25/09

When you choose by clicking Entry you will be advanced to part 3, the payment section of this
entry form. As you complete the payment section of this form you will see your payment receipt number. 
You should print or copy the number,  a copy of the receipt will also be E mailed to you.
